Ohio-North State Referee Committee will manage referee abuse reports in our area. Please reach out to your assigner and complete the Google Form at the button link above.
Provide this information in your report:
- Your name
- Who you have already contacted
- Your position at the game: Center, AR1, AR2
- Date, time, and site/field of the game
- Home and visiting team names
- Age and gender of game
- Who is being reported: player, coach, spectator, etc.
- Category of abuse – see definitions
- Verbal taunting
- Intimidation
- Threats
- Discriminatory acts
- Physical abuse
- Description of incident. Be specific (time in the game, offending person, language or action)
- Supporting documentation 1
- Are you able to provide names, contact information, or description of other(s) that may have seen the incident?
